Is ISO 9001 certification necessary to apply these patterns?

No. The patterns — controlled nonconformity capture, named accountability, root-cause action, feedback loops — are good operational practice independent of certification. Certification adds external auditing and formalisation; the underlying patterns are valuable on their own.

How do we tell performative from usable in our current system?

A practical test: ask when the last exception captured by the workflow led to a documented change in process or product. If the answer is 'I don't know' or 'I can't recall', the workflow is closer to performative than usable.

What is the cheapest first step?

Add structured exception capture to one existing checklist, and commit to a monthly review of those exceptions with the team that produced them. That alone shifts the workflow toward usability before any tooling change.
